All Sport One Day - Waiver Forms
All children participating in All Sport One Day sessions are required to have their parent/guardian sign the Sport Calgary Waiver Form. If you registered online, this has already been completed.

DROP-INS: A copy of the Waiver Form can also be downloaded and printed here for those planning to drop-in at independent locations.
Children participating in specific sessions or at specific locations may be required to sign additional waiver forms provided by the sport organizations and/or facilities.
